• First Name
• Last Name
• Phone Number
• Billing Address
• Account Number
• Date of Birth
• Amount of Last Charge
• Password
• Username
• Email Address
• City
• State/Province/Region
• ZIP/Postal Code
• Country
• Reason for Cancellation
• Last 4 Digits of Card
• Date of Last Charge

Follow these steps:

1. Type or write a letter and be sure to be legible and request cancellation within it. Provide all of your account info as listed. Sign and date the letter and Keep a copy for your records.
2. Make sure to use ceritified mail and send your letter to the address provided to you by the gym
3. Using either a mobile or landline phone, call the phone number for the gym you signed up at or attended most frequently after your letter is received to ensure your account is cancelled
4. Ask the representative for a confirmation number or email
5. In order to make sure you don’t have any issues in the future, keep your confirmation number or email as proof

2. Inquire about how someone can cancel their account
